PRIMARY PURPOSE Maintains oversight for care and services provided by long term care facilities acquired through the Minimum Payment Assistant Program (MPAP) and the Quality Incentive Payment Program (QIPP). Collects and analyzes nursing facilities clinical and non-clinical data for compliance to local, state and federal guidelines. Maintains collaborative relationships and contracts with skilled nursing facilities, long term acute care, inpatient rehab, home health and community based services that are not part of the Parkland System. Develops post-acute initiatives geared toward delivering quality care at lowered cost. MINIMUM SPECIFICATIONS Education: Must have a Bachelor's degree in Nursing. Prefer advanced degree in nursing, business or other related fields. Experience: - Must have 6+ years of hospital or community based patient care nursing, preferably in assigned clinical area. - Must have 5+ years of Long Term Care experience. - At least 2+ years working with the most recent version of the Resident Assessment Instrument and Minimum Data Set, Quality Assurance Performance Improvement initiatives, Medicare and Medicaid Claims processing. - Nursing home surveying experience strongly preferred. - Multi-site home health and hospice experience strongly preferred. - Healthcare administration experience preferred. - Broad regulatory knowledge of home health, nursing home, skilled nursing facility and hospice agency regulation and administration. Equivalent Education and/or Experience: - May have an equivalent combination of education and experience to substitute for both the education and the experience requirements. Certification/Registration/Licensure: - Must have current, valid RN license or temporary RN license from the Texas Board of Nursing; or, valid Compact RN license. - Must have current healthcare provider CPR course completion card from one of the following: American Heart Association American Red Cross Military Training Network Preferred Certifications: Surveyor Minimal Qualification Test Certification, (SMQT), Quality Indicator Surveyor (QIS) Certification, Accreditation for Care Management (ACM) from American Care Manager Association, American Nurses Credentialing Center (ANCC) Nursing Case Management Certification, or National Academy of Certified Care Managers (NACCM).
