The Home Health PT Weekend, under the general supervision of the Care Manager, provides therapy to assigned patients and directs therapy assistants and Home Health Aides to provide quality care in accordance with physician orders under a plan of care. Instructs other health team personnel including, when appropriate, home health aides and caregivers in certain phases of physical therapy with which they may work with the patient. Selects, applies or modifies skilled intervention to the patient using various physical therapy procedures and techniques to achieve the best outcome possible for the patient. (Ex: therapeutic exercises/activities, neuromuscular re-education, gait training, wheelchair management, other modalities such as ultrasound, TENS application, electrical stimulation, hot/cold packs, etc.) Assesses patient response to interventions and performs reassessments as required. Documents in medical record, accurately reflecting treatment provided. Updates the care plan as needed including the aide care plan; writes modified orders for changes in the plan of care, performs and documents the aide supervision by the due date. Completes all documentation within the time frame required by the agency, including reassessments, discharge summaries and OASIS documents. Participates in clinical record review, agency staff meetings, therapy meetings and other activities as requested. We are currently seeking an experienced: HOSPICE LICENSED PRACTICAL NURSE (LPN) Part Time (Burlington/Durham) Job Description: · Provides direct patient care, to include using a variety of skills and necessary coordination of services. · Completes appropriate documentation as required by policy and procedure and regulation. · Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse. · Coordinates Hospice services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians. · Accepts and transcribes physician’s orders as directed. · Forwards all clinical, progress, and summary notes to the Hospice Registered Nurse for coordination of care. · Attends and participates in IDT meetings. Job Requirements: · Successful completion of nursing education from an approved school of nursing. · Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience, preferably in an acute care medical/surgical setting. · Ability to independently organize work and provide effective health care services to hospice patients. · Ability to ensure appropriate patient care as supervised by the Hospice Registered Nurse, and document thoroughly, accurately, and completely on a timely basis. · Effective communication skills (oral and written). · CPR certification. Additionally, there may be some care plans that require daily therapy visits to enhance recovery and reduce rehospitalizations. The Home Health PT Weekend, under the general supervision of the Care Manager, provides therapy to assigned patients and directs therapy assistants and Home Health Aides to provide quality care in accordance with physician orders under a plan of care. Assesses, plans, implements, evaluates total patient care and coordinates patient/family teaching. Assures compliance with therapy standards, federal, state, and local government regulations, and company clinical policies. Provides goal directed care leading to specific outcomes. May be on call for after-hours work. Coordinates and directs the home care patient’s care based on individual patient needs. Workdays will be mutually determined between employee and agency Director; and will exceed minimum requirements for an exempt status professional employee. The value you’ll bring to the team: Performs an evaluation, assessing ability to function safely using a method which objectively measures muscle strength, ability to safely function in the home related to walking on flat, uneven surfaces and stairs, balance, transfers, use of assistive devices, pain management, mental and cognitive factors, and safety issues for fall prevention. The measurement results are documented in the clinical record and updated with each visit. Provides hand-off report to assigned homecare therapy case manager who will be assuming care if the patient. • Periodically reassess the patient every 30 days: provide the ordered therapy service, functionally reassess the patient and compare the resultant measurement to prior assessment measurements. Document the measurements in the clinical record along with the therapist's determination of the effectiveness or therapy or lack thereof. Observes and records activities and findings in the clinical record and reports to the physician the patient's reaction to treatment and any changes in patient's condition, or when there are deviations from the plan of care. Instructs other health team personnel including, when appropriate, home health aides and caregivers in certain phases of physical therapy with which they may work with the patient. Selects, applies or modifies skilled intervention to the patient using various physical therapy procedures and techniques to achieve the best outcome possible for the patient. (Ex: therapeutic exercises/activities, neuromuscular re-education, gait training, wheelchair management, other modalities such as ultrasound, TENS application, electrical stimulation, hot/cold packs, etc.) Assesses patient response to interventions and performs reassessments as required. Documents in medical record, accurately reflecting treatment provided. Updates the care plan as needed including the aide care plan; writes modified orders for changes in the plan of care, performs and documents the aide supervision by the due date. Completes all documentation within the time frame required by the agency, including reassessments, discharge summaries and OASIS documents. Participates in clinical record review, agency staff meetings, therapy meetings and other activities as requested.
