General Description:
Under general supervision, provide social services to patients and their families at OU Health.
Essential Responsibilities:

Provides social services to patients and their families for an assigned medical service
Conducts discharge planning evaluations of patients and/or family members to gather needed information
Makes psychosocial assessments and relates findings to necessary hospital staff

Conducts high social risk case findings and provides crisis counseling to patients and families regarding child abuse or neglect, physical abuse, etc. and documents these sessions

Acts as a liaison between patients/families and the hospital staff and other agencies
Utilizes available information to evaluate the effectiveness of services provided in producing the desired outcomes and makes adjustments in patient care are indicated
Assists in discharge planning and referrals to the community for ongoing services
Assists the patient and family with post hospital placement or transfer to post-acute care facility
Assists patients and families with adverse reactions or difficult adjustments to illness, treatment or changes in functional status
Utilizes specialized interviewing and communication skills with children
Acts a patient advocate for issues related to informed decision making, end-of-life care, realistic goal setting and appropriate resolution of ethical dilemmas
May prepare statistics concerning referrals, extended stay and other elements
May supervise students or newly hired employees

Level II Minimum Qualifications:
Education: Master’s Degree in Social Work.
Experience: 3-5 years of experience required. Hospital experience preferred.
License(s)/Certification(s)/Registration(s) Required: LMSW strong preferred.

Education: Master’s Degree in Social Work.
Experience: 5 or more years of experience required. Hospital experience preferred.
Certifications/Registrations Required: Current Licensed Master Social Worker (LMSW) from the Oklahoma State Board of Licensed Social Workers required, or higher.
Knowledge, Skills and Abilities:
Knowledge of the principles and practices of social and psychological casework, of social and welfare resources available in the community and hospital, of current social and economic conditions, trends and problems. Basic Purpose
The Advocate General within the Office of Juvenile Affairs oversees a comprehensive grievance program for children who have been legally placed under the department's custody. This position will ensure the appropriate reporting of abuse or neglect allegations concerning juveniles in custody, both in private and office-operated facilities. The Advocate General will collaborate with the Executive Director to establish a system for investigating allegations of misconduct by those responsible for children in the custody of the Office of Juvenile Affairs, and coordinate hearings or meetings of review committees for unresolved grievances or investigations. Additionally, the position will have opportunities to provide training internally and externally on policies related to the safe and proper care for youth in OJA custody.
Key Responsibilities
Supervises personnel at children's institutions and facilities as student defender/representatives.
Investigates unresolved juvenile and staff grievances related to juveniles and sends unresolved complaint reports to the Office of Juvenile Systems Oversight for the Executive Director’s review.
Provides recommendations and reports on grievance procedures, hearings, and investigations to the Executive Director, Office of Juvenile System Oversight, and others as necessary.

Job Summary:
The position serves as a support role to the MSW(s) and shall be instrumental in coordinating social supports and community resources for participants as well as participate in team discussion and explanation of the care plan document. This role may also support other roles as needed, including quality initiatives focused on improving services delivered to PACE participants or arranging for transitions of care for PACE participants as directed by the MSW &/or PACE Director.
Duties/Responsibilities:
Serves as assistant to the MSW(s) assisting with set up of life-alert systems and following up with appropriate documentation in chart and participant communication.
Finds and obtains access to community resources, as directed by the MSW, to support participants and their families.
Responsible for completing and appropriately documenting the “Plan B” document, designed to facility the identification of community support available to the participant.
Scans documentation into the chart as directed by the MSW.
Facilitates family meetings or conversations with participants as needed and liaises between participant, families and IDT as needed.
May provide support to other departments as needed and as directed by PACE Director.
Any and all other duties and responsibilities as assigned.
Education, Licenses, Certifications and Experience:
3-5 Years of Social Services Experience
1 Year working with the frail and elder population
High school diploma or equivalent.

GSP Health is looking for a Part-time Behavioral Health Therapist at our Canton location.
The Therapist is a member of the interdisciplinary health team at Great Salt Plains Health Center and works in collaboration with the Director of Behavioral Health, and GSPHC providers. The Therapist provides medical providers with consultation as needed and provides behavioral health care for clients in collaboration with other center providers.
QUALIFICATIONS:
A. Graduate of a formally accredited school with a minimum degree of Master of Science.
B. Must be fully licensed by the Oklahoma State Board of Behavioral Health.
C. Interest and/or experience in collaboration with interdisciplinary health teams.
D. Possesses counseling skills at a high level of competency.
E. Possesses an Oklahoma LPC, LMFT, or LADC license.
RESPONSIBILITIES:
Characteristic Activities:
A. Participates in the provision of behavioral health patient care, including intake assessments, treatment planning, therapy services, discharge, and referral services.
B. Assume responsibility for the mental health services, and maintenance of patient records, for their assigned GSPHC patients.
C. Participates in the coordination of community agencies and/or resources in providing continuity of care.
D. Participates in the continuing education provided by or with the other behavioral health staff, and other GSPHC providers as requested.
E. Provides individual, marital, family, and group therapy services.
F. Consults with other GSPHC providers when patient care requires collaboration and integrated care.
G. Participates appropriately in health center staff activities.
H. Attends all monthly Behavioral Health Team meetings, participating in collaborative professional consultations and didactic trainings.
I. Implements all GSPHC policies and procedures to ensure compliance with existing standards, practices, and funding sources regulations.
J. Participates in obtaining required continuing education.
Specific Functions:
A. Establish positive patient rapport while obtaining all required biopsychosocial information to produce initial intake assessment.
B. Obtain patient signatures required for the Informed Consent to Treatment form and any necessary PHI Release of Information Requests.
C. Establish an informed diagnosis and patient treatment plan utilizing established best practices.
D. Manage patient crisis situations based on established policies and procedures, and according to ethical practices.
E. Makes appropriate referrals to other resources when needed.
F. Determine, in collaboration with the patient, when therapy goals have been met and termination of therapy services appropriate.
G. Participate in clinical audits.
H. Maintain required patient information in patient’s electronic records, accurately and within the expected time frame.
I. Perform other related duties incidental to the work described herein or as assigned by the Director of Behavioral Health.
